HOW TO MAKE A
REFERRAL
1. Complete the referral form and compile the require supplemental documentation, then email it toScummings@Healix.net or fax to 810-953-8704.
2. Our support team will send an acknowledgment of receipt and will notify the patient that we are initiating the approval process.
3. Once insurance is approved, our support team will notifyyou and will contact the patient to review any financialresponsibility and schedule their visit.
Your peace of mind is important to us. Lab work will be returned to your practice, and after each patient appointment, you’ll receive a progress note so you’re always kept informed.
